WOW, you failed at getting a very simple plot. Babydoll and Co. ARE at danger at their fantasies, since they simply reflect what is actually happening. Did you even see the train sequence at all? Oh, and the doctor isn't corrupt at all.

Besides, there is of course a connection and reflection between the brother fantasy and the real world, and how the fantasy matches or does not match the reality is a very important part of the plot. I do agree that the characters were fairly unidimensional, though, and the acting was really poor. Out of the girls, only Vanessa and Abbie could hold their own. The other three are a joke.

I am appalled that you failed at understanding the movie, honestly!
